Training to include, but not limited to: -Truck safety related topics and workplace related topics; -Evaluation of the operator's performance in the workplace. Certification is to include, but not be limited to, the name of the operator, the date of the training or the evaluation, the name of the person providing the training or evaluation, etc. Condition noted on or about 2/25/2016.
