29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    (a) On February 24, 2016; the employer is failing to protect employees from serious struck-by hazards associated with possible breakage of a pedestal grinder from an abrasive wheel. The work rest was not adjusted to 1/8th of an inch or less from the periphery of the wheel on right side; it was adjusted to ¼ inch where employees in the maintenance department work.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  (a) On February 24, 2016, the employer is failing to protect employees from serious struck-by hazards from the tongue guard on a pedestal grinder was improperly adjusted to 3/8 inch; which is in excess of ¼ inch. The employees in the maintenance department are exposed to this hazard from a pedestal grinder located on a work bench.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    (a) On February 24, 2016, the employer is failing to protect employees from electrical hazards throughout the work place. The most recent incident was located in the kitchen and electrical room/maintenance office, where employees had different electrical panels blocked by equipment not provided sufficient access for safe operation and maintenance work.
