29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. (a) On or about February 25, 2016, at the MCAS Miramar worksite, employees installing photo voltaic panels on the roof of Hangar 5 were exposed to falls 17 feet to the lower level.

29 CFR 1926.502(f)(2)(ii): The rope, wire, or chain was not rigged or supported in such a way that its lowest point (including sag) was not less than 34 inches (0.9 m) from the walking/working surface and its highest point no more than 39 inches (1.0 m) from the walking/working surface. (a) On or about February 25, 2016, at the MCAS Miramar worksite, the warning lines on the lower roof of Hangar 5 were sagging below 34 inches and the flags were contacting the walking and working surface. Employees were not protected from falling approximately 43 feet to the lower level.

29 CFR 1926.502(h)(1)(v): The safety monitor had other responsibilities which could take the monitor's attention from the monitoring function: (a) On or about February 25, 2016, at the MCAS Miramar worksite, the safety monitor was performing other duties, such as but not limited to handing tools to an employee and holding photo voltaic panels. This exposed employees to fall hazards of at least 17 feet to the lower level.
