29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  Employees of Narsimha Borra without fall protection were engaged in stucco activities from a pallet attached to elevated forks of an extensible powered industrial vehicle and exposed to fall hazards in excess of six feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  Employees of Narsimha Borra had not been trained to recognize hazards related to falls and the procedures to follow that would minimize those hazards while performing roofing activities, at a height of over 6 feet, without fall protection.

29 CFR 1926.602(c)(1)(ii): Modifications or additions affecting the safe operation of the equipment were  made without the manufacturer's written approval: Note:  If such modifications or changes were made, the capacity, operation, and maintenance instruction plates, tags, or decals should have been changed accordingly.  Employees of Narsimha Borra, performing stucco activities, were exposed to struck-by hazards while operating a powered industrial forklift with an unauthorized wood pallet attached to the forklift.

Employees of Narsimha Borra, performing stucco activities, were exposed to struck-by hazards while operating a defective powered industrial forklift that had not been taken out of service.
