29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about 02/23/2016 - Mill #2, Hydraulic Press #1, lockout/tagout was not utilized by employee performing maintenance on a Betenbender hydraulic press. b) On or about 02/23/2016 - Mill #1 and #2, employees performing clean-up operations at the end of their shifts were not utilizing lockout/tagout prior to placing their hands in the roll form section of the mill.
