29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section.   In the molding production area,  the employer did not provide a specific procedure to control the over pressurization to the mold machine when it malfunctioned exposing employees to struck-by hazards.

29 CFR 1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the procedure.   In the molding production area,  the employer did not train the employees on the hazards when there was a change of operation prior to running a new chemical product in the extrusion mold machine that caused a malfunction and over pressurization exposing employees to struck-by hazards.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident.    In the molding production area,  the employer did not label the electrical panels with the voltage ratings exposing employees to electrical hazards.
