29 CFR 1926.451(a)(1): Each scaffold and/or scaffold component was not capable of supporting, without failure, its own weight and at least 4 times the maximum intended load applied or transmitted to it: On or about February 23, 2016, at a worksite located at 12305 Bailey Court East, North Jackson OH 44451, the employer exceeded the load imposed on a mobile scaffold (scissors lift) guardrail by allowing employees to climb on the guardrail to access the roof to perform work activities.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about February 23, 2016, at a worksite located at 12305 Bailey Court East, North Jackson OH 44451, during commercial low slope roofing activities, the employer did not ensure employees working on the roof were protected from fall hazards of approximately 32 feet, by ensuring a guardrail system, safety net system, personal fall arrest system, or a safety monitoring system was being used.
