29 CFR 1903.19(d)(1): The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.   Abatement documentation was not received for the following:   (a)  On or about February 23, 2016, employees were exposed to fall hazard of approximately 16 feet high while cleaning windows on a low slope roof;   (b)  On or about February 23, 2016, employees were exposed to workplace hazards while cleaning windows.  Employees were exposed to fall hazards, eye hazard and struck by hazards;   (c)  On or about February 23, 2016, employees were not provided PPE training for fall protection for rappelling operations or other PPE used during window cleaning;   (d)  On or about February 23, 2016, the employer did not ensure a written hazard communication program was maintained at the facility.  Employees were required to use chemicals including, but not limited to, cleaning liquids including dish detergent and pumice scouring powder to clean windows;   (e)  On or about February 23, 2016, the employer did not ensure all containers or chemicals leaving the facility were properly labeled.  These chemicals include, but are not limited to, Dawn detergent and pumice scouring powders;   (f)  On or about February 23, 2016, the employer did not ensure a safety data sheet was available for each hazardous chemical in the workplace.  Hazardous chemicals include, but are not limited to, Dawn detergent and pumice scouring powders;   (g)  On or about February 23, 2016, the employer did not ensure all employees were trained on the chemicals they use and are exposed to.  Chemicals include, but are not limited to, Dawn detergent and pumice scouring powders.
