29 CFR 1926.252(a): Whenever materials are dropped more than 20 feet to any point lying outside the exterior walls of the building, an enclosed chute of wood, or equivalent material, was not used:    a. On February 25, 2016, on the 6:12 pitch roof, employees are throwing tile from the roof to the ground 22 feet below, exposing employees to the hazard of being struck by the tile.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:    Employees performing roofing work on the 6:12 pitch roof are not protected from a 22 feet fall hazard by the use of a fall protections system.    The employer fails to assure effective protection against falls as required by the standard, as demonstrated on February 25, 2016, when employees were working on a residential roof at 102 Costa Del Sol, Saint Augustine, Florida 32085 without fall protection and were exposed to a 22 feet fall hazard.    Collis Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13)), which was contained in OSHA inspection number 721522, citation number 2, item number 1 and was affirmed as a final order on February 3, 2014, with respect to a workplace located at 204 38th Avenue South, Jacksonville Beach, Florida 32250.
