29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices: a. Engine Room- The employer failed to document process safety information for relief valves RV10, RV15, RV20, RV25, and RV28 to ensure that the relief valves were the proper type, size, and set pressure for protecting against an overpressure condition of thermosyphon oil coolers on screw compressors RC1, RC3, RC4, RC5, and RC6. b. Roof- The employer failed to document that the ammonia relief vent discharging to atmosphere in a downward direction 10 feet from the roof surface between two roof top evaporative condensers and below the catwalks near the top of the two evaporative condensers complied with a recognized and generally accepted good engineering practice. The employer stated in their written program that they are adhering to ANSI/ASHRAE 15-2004 and verbally communicated that they are adhering to ANSI/IIAR 2-2014 in reference to ammonia relief vent discharge. The employer failed to comply with either standard.

29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before use: a. Engine Room- The employer failed to remove and replace relief valves RV10, RV15, RV20, RV25, and RV28 where the relief valves were protecting against an overpressure condition of liquid in the thermosyphon oil coolers on screw compressors RC1, RC3, RC4, RC5, RC6 and were not designed for liquid. b. Roof- The employer failed to protect employees from exposure to ammonia where the ammonia relief vent was discharging to atmosphere in a downward direction 11 feet from the roof surface between the two roof top evaporative condensers and below the catwalks near the top of the two evaporative condensers.
