29 CFR 1910.213(a)(12): The portion of circular saw(s) beneath or behind the table, where there was a possibility of contact with the saw, was not covered with a guard or exhaust hood:       The employer is failing to ensure that an adequate guard was used on the DeWalt Radial Saw. There was not an adequate guard behind the saw when it returned to its housing position.  An employees right hand was amputated when it came in contact with the saw blade.       Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.
