(b) Inside house rear staircase:  The rear staircase did not have a stair rail system protecting employees from falling off the unprotected side.

29 CFR 1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:  Jobsite:  Pump jack scaffolding was not erected, dismantled, or altered by trained employees or under the supervision of a competent person.  The scaffolding was not erected and braced in accordance with manufacturers requirements.   Columbia Contracting Corporation was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 315556522, citation number 1, item number 2, and was affirmed as a final order on October 3, 2011, with respect to a workplace located at 45 Highland Street, Cambridge, Massachusetts.
