29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:  a) Facility, 294 North Industrial Park Road, Milton, PA - Employees lifted extruded metal with a Caterpillar Lift Truck Model No. GC45KS1 (Serial No. AT87A01248) modified with a Koke Lift Jib (Model LJ-180-2K) attached to the forklift tines without the manufacturer's prior written approval exposing employees to struck-by and fall hazards created by the modifications to the powered industrial truck's lifting capacity and center of gravity, on or about February 26, 2016.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation.
