29 CFR 1926.301(a): Unsafe hand tool(s) were issued or permitted to be used:   On or about 24 February 2016, at MCAS Miramar Building 9211 the employer failed to ensure that all equipment was inspected prior to use. The built in indicator light on the plug end of the RIDGID corded power drill was damaged and in use, exposing employees to electrical hazard.

29 CFR 1926.405(j)(1)(i): Fixtures, lamp holders, lamps, rosettes, or receptacles had live parts normally exposed to employee contact:  On or about 24 February 2016, in the office space under renovation at MCAS Miramar Building 9211, the employer failed to ensure that outlets used to power a vacuum and a mixer, were covered by a cover plate. Employees were exposed to live electrical parts of more than 50V.
