29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) SPS Technologies, Department 6670 - On or about February 9, 2016, an employee was exposed to pinch point hazards of a 60 Waterbury R24 Thread Roll Machine where one or more methods of machine guarding was not provided to protect the operator from hazards.    b) SPS Technologies, Department 6670  - On or about February 26, 2016, an employee was exposed to pinch point hazards of a 60 Waterbury R40 Thread Roll Machine where one or more methods of machine guarding was not provided to protect the operator from hazards.    ABATEMENT NOTE: Among other methods, one feasible and acceptable abatement method to correct this hazard is: Comply with ANSI B11.7 Cold Headers & Cold Formers - Safety Requirements for Construction Care & Use, B11.19 Performance Criteria for Safeguarding. Additional methods is to also develop and implement a Safe Distance Safeguarding Program and Exposure Prevention Procedures. Specialized training and supervisorial monitoring of employees is essential to ensure that safeguarding methods and exposure preventative procedures are being used properly.  Safe work procedures are formal, written instructions which describe how a task is to be performed. These procedures should incorporate appropriate safe work practices, such as prohibiting employees from wearing loose clothing or jewelry and requiring the securing of long hair with nets or caps. Clothing, jewelry, long hair, and even gloves can get entangled in moving machine parts.  Using hand tools requires close supervision to ensure that the operator does not bypass their use to increase production. It is recommended that these tools be stored near the operation to promote their use.  To prevent injury and repetitive trauma disorders, hand-feeding tools should be shatterproof and ergonomically designed for the specific task being performed.
