Fandrey Construction, LLC, 4995 Station Road, Erie, PA 16510, on or about February 26, 2016 and times prior thereto: Employees were utilizing a Ridgid miter saw for work operations including, but not limited to, cutting vinyl siding and trim. Employees were operating the saw without the lower guard in place.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage: A.) Fandrey Construction, LLC, 4995 Station Road, Erie, PA 16510, on or about February 26, 2016 and times prior thereto: A flexible electrical extension cord was powering equipment including, but not limited to, a Makita Circular Saw (M/N: 5007NB). The flexible electrical extension cord was running under a sliding wooden door. The cord was not protected against physical damage created by the siding door.
