29 CFR 1926.453(b)(2)(iv): Employees shall always stand firmly on the floor of the basket, and shall not sit or climb on the edge of the basket or use planks, ladders, or other devices for a work position.     (a)Architectural Roofing & Sheetmetal, Inc. @ 301 South Main, Sioux Falls, SD 57104:  On or about February 24, 2016, the employer did not ensure employees were protected from a fall hazard associated with an employee working outside of the aerial lift personnel platform. An employee engaged in the installation of a new roof on the Washington Pavilion was working outside the platform, while his fall protection device remained secured to the guardrail of the aerial lift. This hazard exposed the employee to a fall of approximately 16 feet.

29 CFR 1926.502(d)(8): Horizontal lifelines shall be designed, installed and used, under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintains a safety factor of at least two:         (a)Architectural Roofing & Sheetmetal, Inc. @ 301 S. Main, Sioux Falls, SD 57104:  On or about February 24th, 2016, the exposing employer did not ensure that a horizontal lifeline system was designed, implemented, and tested to ensure the specific configuration of the lifeline, including amplification of forces due to the angle of sag, and maximum arresting forces involving the load of one employee imposed on the lifeline, would support that employee in the event of a fall event. The horizontal lifeline system consisting of a 5/8" diameter synthetic rope was anchored or looped around existing structural components which presented sharp edges that contacted the rope. An employee was connecting his lanyard snap hook directly to the synthetic rope while working to install a new roof on the Washington Pavilion.  This condition exposed an employee utilizing the lifeline to a fall hazard of approximately 50 feet, due to the potential failure of the system in the event of a free fall.
