29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:  One Wallace Ave., South Portland, Maine, welding bay - On or about March 1, 2016, employees  performing welding activities had time-weighted average noise levels of 89.1, 89.7, 86.7, and 89.1 decibels measured on the A scale (with respective doses at 88.9, 95.9, 63.8, and 88.5%), and a monitoring program had not been developed.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:  One Wallace Ave., South Portland, Maine, welding bay - On or about March 1, 2016, audiometric testing was not made available to employees exposed to noise levels exceeding 85 decibels (for example, but not limited to, 89.1 and 89.7decibels) while performing welding activities.
