CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).      (a) Packaging Corporation of America @ 4221 Monaco Street, Denver, CO 80216.  On or before December 17, 2015, as part of the injury/accident prevention responsibilities under its safety program, the employer did not ensure that each employee who was allowed to operate powered industrial trucks was trained to operate such vehicles, including, but not limited to, the Rico Walkie-Rider Roll Cart.  A Corrugator Stacker Operator operated a Rico Walkie-Rider Roll Cart, model No. R-LPRH-WR-80 Forklift, without adequate training in accordance with this standard.  This condition exposed employees to struck-by hazards from improper forklift operations.      Abatement Note:  Training shall consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.

29 CFR 1910.178(q)(1):  Powered industrial trucks with defects or in any way unsafe had not been withdrawn from service until restored to safe operating condition(s). All repairs shall be made by authorized personnel.       (a) Packaging Corporation of America @ 4221 Monaco Street, Denver, CO 80216.  On or before December 17, 2015, the employer did not ensure that powered industrial trucks, including, but not limited to, the Rico Orange Walkie-Rider Roll Cart, were free from defects before use.  The Rico Orange Walkie-Rider Roll Cart had a  balancing wheel under the fork that had been worn down while transporting paper rolls in the corrugator area.  The balancing wheel had not been repaired prior to use.  This condition exposed the employees to struck-by hazards caused by an unsafe forklift operation.
