29 CFR 1910.305(a)(2)(x): Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points. On or about February 29, 2016 at or near 8200 Frazier Pike, Little Rock, AR 72206, employees were using an extension cord plugged-in in the break room and ran to the work floor beneath the doorway. The extension cord was not protected from accidental damage. This condition exposed employees to electric hazards.

29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, and service rooms shall be kept clean and orderly and in a sanitary condition. On or about February 29, 2016 at or near 8200 Frazier Pike, Little Rock, AR 72206, employees were using an extension cord plugged-in in the break room and ran to the work floor across a passageway. The extension cord was raised approximately 2 inches on the passageway. This condition exposed employees to trips/fall hazards.
