29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed:      a)  On February 29, 2016, the extension ladder was used by two employees simultaneously, to access and/or egress a ladder jack scaffold platform.         In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:      a) On February 29, 2016, employees were exposed to the hazard of falling while working from a ladder jack scaffold platform elevated approximately 16 feet above the ground.       Kozik General Construction, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(i), which was contained in OSHA inspection number 933669, citation number 1, item number 2 and was affirmed as a final order on October 29, 2013, with respect to a workplace located at 145 East Ave., Oak Park, Illinois 60302.    No abatement certification or documentation is required for this item.
