29 CFR 1910.178(a)(5): The truck was equipped with front-end attachments other than factory installed attachments, and the user did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered.  a) Barrel warehouse:  On or about February 24, 2016 and at times prior thereto:  A Hyster, Electric Lift Truck Model J808, SN B160V01829H, was observed with a front-end attachment, Model DGS-55-D, Vestil Manufacturing Corp., used for moving product barrels weighing approximately 700 pounds on average.    Note* Documentation of abatement IS required for this item.

29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation.  a) On or about 24 February, 2016 and at times prior to; the operator of the Hyster, Electric Lift Truck Model J808, SN B160V01829H had not been formally evaluated by the employer to operate a forklift and hands-on portion of the training was not documented.   b) On or about February 24, 2016 and at times prior to; the operator of the Hyster 30, Model H30XM, SN D001H05688X, had not been formally evaluated by the employer to operate a forklift and hands-on portion of the training was not documented.
