29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries:   (a) On or about February 24, 2016, employees were exposed to possible fire hazards in that access to a fire extinguisher mounted on the west wall of the shop, toward the south end, was blocked by a shop desk with a backboard.   Abatement certification is required for this item.  Use the Certification of Corrective Action Worksheet for this purpose.

29 CFR 1910.178(l)(1)(i): The employer did not ensure that each powered industrial truck operator was competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):      (a) On or about February 24, 2016, and at certain times before, an employee was allowed or required to operate a "Hyster 60 Fortis" powered industrial truck.  The employee had not been trained or certified in that the employer had no training or certification program as required by standard.       Abatement certification is required for this item.
