Powered industrial truck operators shall receive initial training in the following topics; operating instructions, warnings, and precautions for the types of truck the operator will be authorized to operate. (a) Jobsite - On or about March 01, 2016 employees attached a wooden trash box to a JLG G10-55A all-terrain forklift to be used as a work platform.

29 CFR 1910.178(l)(6): At this establishment, the employer did not certify that each operator has been trained and evaluated to operate the powered industrial truck as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. (a) Jobsite - On or about March 01, 2016 forklift operators have not been certified by the employer.
