Site - On or about February 29, 2016, an access ladder was not provided for the ladder jack scaffolding system in use by employees.

29 CFR 1926.451(g)(1): 29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1m) above a lower level was not protected from falling to that lower level by fall protection established in accordance with paragraphs (g)(1)(i) through (vii) of this section: a. Ladder scaffolding - On or about February 29, 2016, employees engaged in roofing activities while working from the scaffolding system were not wearing fall protection and were exposed to falls of over 11 feet.
