29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to items (a) through (d) of this section:  Machine shop area and throughout the plant - Employees engaged in maintenance activities were exposed to electrical, struck-by and caught-between hazards in that all of the energy control procedures describe how to de-energize the equipment.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  Machine shop area by the 3-Axis CNC - Employees engaged in machining activities were exposed to struck-by and amputation hazards in that machine guarding was not provided and utilized for the Mori Seiki milling machine #1355, MV-80, Serial #136. The employer had installed a new door that did not prevent employees from reaching the point of operation while the machine was in operation.

29 CFR 1910.334(a)(2)(i): Portable cord- and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence for possible internal damage (such as pinched or crushed outer jacket):    Machine shop area by the 3-Axis CNC - Employees engaged in machining activities were exposed to electrical shock and fire hazards in that the flexible cord supplying power to the liquid pump under the Mori Seiki milling machine #1355, MV-80, Serial #136 had damage to the outer sheath jacket exposing the internal insulated conductors at both ends of the cord.
