29 CFR 1926.451(b)(1): Each platform on all working levels was not fully planked or decked between front uprights and the guardrail supports: (a) North Building No. 3 - On or about March 02, 2016 employees were working from the third level of a masonry scaffold that was not fully planked. The employees were exposed to a fall height of 18 feet.

29 CFR 1926.451(e)(1): scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. (a) North Building No. 3 - On or about March 02, 2016 employees used the supports of a masonry scaffold to access the working level of the scaffolding.The scaffold did not have a ladder attached to access the work area.
