29 CFR 1926.153(o): When damage to LP-Gas systems from vehicular traffic was a possibility, precautions against such damage were not taken: (a) Jobsite, 4800 Penn Avenue, Pittsburgh, PA: On or about 02/29/16 two 1000 gallon propane tanks used for heating the building were not adequately protected from damage from vehicular traffic. A chain link fence was positioned directly next to the tanks and the alley way where vehicles travel and park and would not protect the tanks from damage. In addition, aerial lifts traveled on the inside of the fenced work area in and around the tanks and was only separated by flat pieces of metal sheeting positioned on the ground.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l): Note: The requirements applicable to construction work under paragraph (d) of this section are identical to those set forth at paragraph (l) of 29 CFR 1910.178: (a) Jobsite, 4800 Penn Avenue, Pittsburgh, PA: On or about 02/29/16 an employee who was operating the Sky Trak 6042 powered industrial vehicle had not been trained and evaluated to ensure they were capable of operating the equipment safely.
