29 CFR 1904.29(b)(3): The employer did not enter each recordable injury or illness on the OSHA 300 Log and 301 Incident Report within seven (7) calendar days of receiving information that a recordable injury or illness has occurred:  (a) On or about March 25, 2016 and at times prior to that date, for the employee who suffered an injury on the DoAll metal cutting band saw on February 25, 2016 whose injury was not recorded on the log in accordance with recordkeeping regulations, located at 410 Sixth Street SE, Mandan, North Dakota.
