29 CFR 1926.300(b)(4)(ii): The point of operation of machine(s) whose operation exposed an employee to injury was not guarded: (a) Jobsite, 2454 Henry Road, Franklin Park, PA: On or about 03/02/16 the Skilsaw 3310 table saw used to cut 1 1/2 inch thick BASF Senergy Expanded Polystyrene Thermal Insulating Board did not have a guard to prevent contact with the rotating blade. Approximately 2 1/2 inches of the blade protruded above the table and no guard was in placed to prevent contact with the point of operation.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: (a) Jobsite, 2454 Henry Road, Franklin Park, PA: On or about 03/02/16 employees were working on sections of the scaffold approximately 13 - 19 feet above the ground and no guardrails or personal fall arrest systems had been installed to prevent the employees from falls.
