OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to flash fire/explosion hazards while using a vacuum truck to collect combustible dust composed of steel and graphite. a. On or about March 1, 2016, three employees utilized a Vactor Guzzler vacuum truck to remove an abrasive blasting dust containing steel and graphite from a 55 gallon steel drum. The employer did not employ methods to ensure that the system (drum, hose, fittings, truck components, etc.) were bonded and grounded. In addition, the hose being utilized was not conductive. These conditions led to a flash fire inside the vacuum truck which exited the truck and resulted in 1st, 2nd, and 3rd degree burns to the arms and legs of the employees.
