Location: 372 Broadway 3rd floor  On or about: March 9th 2016  a) Employees working on a tubular welded frame scaffold that did not have all guard rails installed.  Guardrails were not installed in a section of the scaffold and personal fall arrest system was  not used when accessing the area of the scaffold.
