29 CFR 1910.332(b)(1): Employees were not trained in and familiar with the safety-related work practices required by 1910.331 through 1910.335 that pertained to their respective job assignments:(a) On or about February 1, 2016, unqualified employee(s) were required to open live 480 volt electrical panels installed inside of the Lift Station #88016, in order to respond to a lift station alarm, and had not received specific electrical safety work practices training, exposing employees to the hazards of electrical shock.

29 CFR 1910.333(b)(2)(v)(A): A qualified person did not conduct tests and visual inspections, as necessary, to verify that all tools, electrical jumpers, shorts, grounds, and other such devices had been removed, so that the circuits and equipment could be safely energized. On or about February 1, 2016, while troubleshooting a lift station alarm, an unqualified person/employee was directed to attempt to reenergize a 480 volts 3 phase control panel at Lift Station# 88016 without conducting any tests or visual inspections as required, exposing employees to the hazard of electrical shock.

29 CFR 1910.335(a)(1)(i): Employees working in areas where there were potential electrical hazards were not provided with electrical protective equipment that was appropriate for the specific parts of the body that needed to be protected and for the work being performed: On or about February 1, 2016, at lift station #88016, the employer did not ensure that employee(s) troubleshooting a lift station alarm by opening 480 volt panels were provided with electrical protective equipment, such as but not limited to, arc rated face shield and leather gloves, exposing employees to the hazards of electrical shock.
