29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work: a) East side of the Warehouse: Employees operated a Black & Decker model #6820 bench grinder to trim metal pieces, with a work rest that measured 2 inches from the abrasive wheel, on or about 2/4/16. b) East side of the Warehouse: Employees operated a Black & Decker model #6820 bench grinder to trim metal pieces that did not have a work rest, on or about 2/24/16.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) East side of the Warehouse: The peripheral protecting member of a Black & Decker model #6820 bench grinder was measured at 2 ½ inches from the abrasive wheel. The bench grinder was used to trim and modify small metal pieces, on or about 2/24/16.
