Appleton, WI: On March 3, 2016, employees framing and sheathing the roof of an apartment building were exposed to eye injury from flying objects, such as pneumatic gun nails and wood dust, while cutting and nailing without safety glasses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Mr. Fix It project site located at 3500 French Rd. Appleton, WI: On March 3, 2016, employees were exposed to fall hazards while framing a two story apartment building without fall protection while:    A) One employee was standing on the 4:12 hip roof approximately 8' above the ground while cutting OSB sheathing and passing the pieces to the employees on the upper roof surface;    B) Three employees were working on and between the trusses, which were >12' above the interior floor level, while framing and sheathing the roof system;    C) Accessing the truss system from a step ladder positioned next to an unguarded stairwell opening 10' above the ground floor slab;    D) Working around and near two unguarded wall openings which were 10' above the pavement of the parking lot;    E) One of the two lifelines in place but not used was tied to a truss while the other was wrapped around a truss and hooked back onto itself.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Mr. Fix It project site located at 3500 French Rd. Appleton, WI: On and before March 3, 2016, employees had not been trained by a competent person to properly set up and use personal fall arrest systems as evidenced by:    a) a rope lifeline was tied to a wood truss without a snap hook or anchor connector;    b) a rope lifeline was wrapped around a wood truss and hooked back onto itself instead of connecting the snap hook to an anchor;    c) employees were wearing harnesses that weren't adjusted properly with all buckles connected;    d) two lifelines were provided for the four employees exposed to falls;    e) two patio door wall openings did not have guardrails in place.

Appleton, WI 54919: On and before March 3, 2016, the employer did not train each employee in the proper selection and use of ladders as evidenced by:    a) the top half of an aluminum extension ladder was used as a separate piece, without slip resistant feet;    b) the aluminum extension ladder used to access the second floor did not extend at least 3' above the upper surface;    c) a 6' fiberglass step ladder was used to access the roof truss system that was 8' above the floor.
