29 CFR 1910.147(d)(5): Following the application of lockout or tagout devices to energy isolating devices, all potentially hazardous stored or residual energy were not relieved, disconnected, restrained, and otherwise rendered safe: (a) On or about 02/29/2016 - Main plant, Salem Band Head Rig band saw, serial #R9544281, during the change out process with the saw blade, an employee was allowed to sweep in and around the saw blade before the blade had come to a complete stop.
