29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: (a) On or about March 2, 2016, at the Hollywood Casino (Level C1) worksite, the industrial floor fan provided by the general contractor had a power cord that was torn, exposing subcontractors to electrical shock hazards.

29 CFR 1926.404(b)(1)(iii)(C): Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. (a) On or about March 2, 2016, at the Hollywood Casino (Level C1) worksite, the industrial floor fan provided by the general contractor was missing a grounding pin, exposing subcontractors to electrical shock hazards.
