29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use:    a) Jobsite - A wooden pallet attached to a forklift truck was used to raise employees conducting light installation activities to a height greater than 10 feet without the manufacturer's approval. This condition exposed employee(s) to fall hazards.     Noted on or about 2/19/2016.

29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards.  a) Jobsite - Employees were not instructed in the recognition and avoidance of hazards associated with scaffolds, including, but not limited to, falls, and the regulations of their work environment to control or eliminate hazards. The employees were exposed to fall hazards at a height greater than 10 feet while performing light installation on an elevated work platform on or about 2/19/16.
