29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: 19 Butternut St, Greenfield, MA On 3/4/16 and at times prior, the guarding on the Pistorius Vee Nail machine did not prevent the operator from putting fingers beneath the clamp mechanism during the clamping operation. On 1/27/16, an employee received a recordable finger injury when a finger was crushed beneath that clamp mechanism.
