29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch On or about March 4, 2016, at the above addressed jobsite, 6540 N.W. 35th Avenue, the employer did not provide adjustable tongue guard on the Baldor floor grinder having serial number P9-93. This exposed the operator of the grinder to stone wheel fragments in the event of an exploding wheel. Associated Machine Co., Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard CRF 29 1910.215(b)(9), which was contained in OSHA inspection number 315355925, citation number 1, item number 2 and was affirmed as a final order on October 14,2011, with respect to a workplace located at 6540 N.S. 35th Avenue, Miami, Florida 33147.
