Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification: a. On or about March 03, 2016, at the above addressed jobsite, power cords used to operate circular saws (Makita-574ONE) were spliced, which did not conform to the original manufacture's listing and certification, thereby exposing employees to electrical hazards. b. On or about March 03, 2016, at the above addressed jobsite, employees operating circular saws (Makita- 574ONE) were plugged into an outlet power strip which did not conform to the original manufacture's listing and certification, thereby exposing employees to electrical hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems shall be installed along all open sides and ends of platforms. Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews: a. On or about March 03, 2016, at the above addressed jobsite, an employee installing siding from a pump jack scaffold, approximately eighteen (18) feet above the ground, was not provided with a complete guardrail system, thereby exposing the employee to the hazards associated with falls.
