29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: On or about March 4, 2016, the employer was using a damaged extension cord. The outer jacket was cut on two locations and was missing a ground prong. This condition exposed employees to hazards associated with electrical shock.
