29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes:  a/  Inside of the Main Building, operator(s) of the Clark Fork Truck, m/n C500-07, s/n 685-52-2251, were not provided with suitable hand protection when changing out the LPG cylinders.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):  a/  Inside of the Main Building, all four, solid rubber tires belonging to the Clark Fork Truck, m/n C500-07, s/n 685-52-2251, were missing large, chunks of rubber.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a/  Inside of the Finishing Center, the Kirk-Rudy Feeder Unit, m/n 311, s/n 433615, was unguarded.  An amputation occurred on or about 03/05/16.  b/  Inside of the Main Building, the Hogger was unguarded. The moving component of this unit measured approximately 67.5"L X 74"H.  Operator(s) were exposed to a serious pinch-point hazard.  c/  Inside of the Maintenance Department, the rotating chuck belonging to the Yamazaki Lathe, unknown m/n or s/n, was unguarded.  d/  Inside of the Maintenance Department, the rotating chuck belonging to the Sharp Lathe, m/n 1540, s/n W10386, was unguarded.
