29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level:     On or about March 4, 2016 Employees working on the roof bows were exposed to a fall hazard, because the platform did not have a standard railing.  The top rail remained in a lowered position.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section:    On or about March 7, 2016, employees were exposed to unexpected start-up or release of stored energy while performing servicing and maintenance activities on the Hydraulic Press, and the employer did not developed specific procedures that outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy.   The procedures shall clearly outline:     a. A specific statement of the intended use of the procedure, b. Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy, c. Specific procedural steps for the placement, removal and transfer of lockout devices and the responsibility for them, and  d. Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.178(p)(1) - If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition.       On or about March 4, 2016, an employee was exposed to struck by hazards while operating a Komatsu Forklift, and was not taken out of service, until it was evaluated for safe operating condition.  The Forklifts brakes needed attention.
