29 CFR 1910.23(b)(1)(i): Every wall opening from which there is a drop of more than 4 feet was not guarded by one of the following:  Rail, roller, picket fence, half door, or equivalent barrier.   On or about March 4, 2016, employees were exposed to fall hazards when the doors in the warehouse could be opened and there were no longer any steps to the lower level on the opposite side of the wall.  The doors had been marked NO EXIT.  Yellow caution tape had been draped over the doors.  There was no warning to indicate that a fall would occur if one walked through the doors.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about March 4, 2016, employees were exposed to electrical and struck-by hazards when lockout/tagout was not used to perform the following tasks: a.   Changeovers on the casers according to LIT-SAF-03-008 (written by the safety coordinator) b.   Changeovers on the bagger machines according to LIT-SAF-03-008 (written by the safety coordinator).  According to employee interview, employees press the e-stop to change film on the packers. c.   Replacement of gauges, switches on air compressors are considered minor and do not require lockout/tagout according to LIT-SAF-03-008 (written by the safety coordinator) d.   Removing cups from the test cabinet on the cup machines.  Employees are taught to press the e-stop. e.   Removing cups from under the nets.  The net is interlocked and machine stops if the net is lifted. f.    Cleaning the printer and removing cups at the end of the shift.  The door is interlocked.  Employees turn off the machine using e-stop. g.   Walking behind the cup machines in order to add hydraulic oil to the equipment.  Equipment is running while the employee is behind the machines.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about March 4, 2016, employees used materials such as, but not limited to, expandable polystyrene beads (pentane), UV curable ink, propane (building 2) and machine oil.  Employees interviewed said that they did not know where to find SDS or said that the MSDS were in a binder.  MSDS binder was not kept up-to-date.  Up-to-date SDS were available on line.  No one interviewed mentioned finding SDS on-line.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
