29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent) was not completed in the detail as required by the regulation.   On or about 7 March, and at times prior thereto, the employer failed to complete in detail the information required on the OSHA 300 Log of Work-Related Injuries and Illnesses for calendar year 2015 in Column F (describe injury or illness, parts of body affected, and object/substance that directly injured or made person ill). 1) Case# 2 indicated an employee tripped and fell, but not what injury occurred or what object caused the injury. 2) Case# 5 indicated an employee injured themselves during an approved company customer sales event but not what injury occurred or what object caused the injury. 3) Case #6 indicated an employee suffered a head injury when they walked into a 1.5 copper pipe, but not what injury occurred, such as a concussion, laceration or contusion. 4) Case# 9 indicated an employee slipped off a ladder banging their knee, but not what injury occurred and to which knee.
