29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a) Roof Structure (Exterior Front): Employees engaged in roofing work on a two story residential structure, were not protected from falling objects, including but not limited to, shingles and tools. The employer failed to provide a hard hat or protective helmet, thus exposing employees to a head injury, on or about, 3/7/16.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a) Roof Structure (Exterior Rear): Employees engaged in roofing work on a two story residential structure, at an approximated height of 30 feet, were not protected from a fall hazard to the lower surface. The employer failed to provide a personal fall arrest system, thus exposing employees to a fall hazard, on or about, 3/7/16.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Roof Structure (Exterior Front): Employees engaged in roofing work on a two story residential structure, were working from a twelve foot aluminum A-frame ladder that did not extend at least 3 feet above the upper landing surface. The employer failed to ensure that employees were properly using ladders, thus exposing employees to a fall hazard, on or about, 3/7/16.
