29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.    On or about March 8, 2016 employees were exposed to rotating parts when the employer did not require the utilization of machine specific energy control procedures for employees performing spooling over run recovery operations of the #5 and #6 spooling machines. The energy sources include, but are not limited to, electrical and mechanical.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    On or about March 8, 2016, employees were exposed to the rotating parts on Spooling machines 5 and 6 when the employer did not ensure employees performed the procedures for the application of energy control by; preparing the machine for shutdown [per the 1910.147[(d)(2)], isolating the machine of all energy sources [d)(3)], affixing lockout devices to hold the machine in the off position [(d)(4)(i)], and verifying that isolation and de-energization of the machine had been accomplished [(d)(6)], during the overrun recovery operations.  The energy sources include, but are not limited to, electrical and mechanical.

The energy sources employees were exposed to include, but are not limited to, electrical and mechanical.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    a) On or about February 26, 2016 employees were exposed to nip points when an employee caught his left thumb in the moving parts on brush turner #1.      b) On or about March 8, 2016 employees were exposed to rotating parts when the employer did not fully guard the rotating parts of spooling machines #5 and #6.    C) On or about March 8, 2016, employees were exposed to rotating parts when the coiler was left unguarded on the front and rear of coiling machine #2.      D) On or about March 8, 2016, employees were exposed to rotating parts when the chuck of the large brush spinner was not guarded.
