29 CFR 1926.451(f)(3): Scaffolds and scaffold components shall be inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity. A) Building exterior, Scaffold: Employees fell approximately 24 feet when the ladder jack scaffold they were working on collapsed. The scaffold used by the employees was not inspected by a competent person prior to commencing work. The scaffold had numerous hazards such as, but not limited to: bent and broken rungs and broken feet on several of the ladders. These same ladders were being used for access by the employees and to support the scaffold that the employees were working from, on or about December 10th, 2015.
