29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a) On or about 03/07/2016, in baghouse 3 cell 5, employer failed to provide employee with personal protective equipment, such as a personal fall arrest system, when employee was attempting to reach and adjust a chain which was holding up a 35 ft filter bag being held in place by angle irons 11 inches apart. Employee fell through the 11 in gap, from a height of approximately 35ft, and sustained multiple fractures. ABATEMENT DOCUMENTATION REQUIRED
